     Full Name

Disposed Equipment Form
Enter
     Equipment Description   i.e. Laptop
     Service tag or serial number (usually found on the bottom of the laptop)
     Price paid (Enter $0 if not money was paid)
     To whom was money paid?

Ticket Details
Issue Summary, enter what you are purchasing i.e. Laptop
Asset Type, leave as Workstation

Select Create Ticket button
